STEP-BY-STEP GUIDE TO SETTING UP YOUR MIND THERAPY CLINIC PORTAL ACCESS
The following items are required to setup your Mind Therapy Portal Account:
The Your Last Name.
The Validation Code that was provided to you from Mind Therapy Clinic. This code expires after 7 days from receipt.
OR: Go to the URL (webpage address) of the Portal Account setup page for Mind Therapy Clinic: https://www.valantmed.com/Portal/mtc
The following steps are the patient's experience while setting up a patient portal account:
Click on “Need to Set Up Your Account?” link under Log On button. The Setup Account page will appear.
Enter your last name in the Last Name field. (The field is NOT case sensitive.)
Enter the Validation Code in the Validation Code field. (This was given to you by MindTherapy Clinic.)
Enter the words that are displayed in the reCaptcha Security Box. (For the visually impaired, click on the speaker icon to hear the display verbally.)
Select the Setup Account Button. The Account Setup Box will appear.
Enter a username of your choosing in the User Name field.
This username will be used to log into the Patient Portal
You will need to remember this username.
Enter an OPTIONAL email address in the Email field.
At the bottom of the Account Setup Box, enter a password in the Password and Confirm
This password will be used to log into your Portal page.
You need to remember this password.
The requirements of the password are as follows:
The password must be at least 6 characters long.
The password must have at least 1 upper case letter.
The password must have at least 1 lower case letter.
The password must have at least 1 number or non-alphabetic character (e.g., !@#$%^&*?).
Next, choose three security questions, which are used to reset your password if it is forgotten.
Select the Create Account Button.
At this point, you have the following choices:
If you enter an email address, click on the Send Receipt to Above Email Button.
This email is not saved in the system.
If you decide not to enter an email address, click on the Proceed without Receipt Button.
If there is an email address on file or if you selected Send Receipt to Above Email on the previous step, the you will receive the following email:
You will be logged in and taken to the Patient Portal homepage. From here, you can immediately begin filling out demographic information and any clinical questionnaires or outcome measures that have been pre-assigned.
